There are currently five types of graduate degrees offered at ESF. Forms are organized by degree program, select your program to skip to that section. All forms are in PDF format unless indicated otherwise.

General Forms

• Full-Time Status Form
  (this form is to be completed by Masters students in their final semester or PhD students who are passed candidacy exams who are enrolled for less than full time AND who are not currently appointed as a Graduate Assistant (GA) or Research Project Assistant (RPA). This form needs to be completed each semester.)
• Petition for General Procedure
• Petition to Revise an Existing Program of Study (Courses and Committee Members)
• Petition for Extension of Time Limit for Degree Completion 
• Re-enrollment Form
  (this form is to be completed by ESF Graduate students who were not enrolled during the immediate past one or two semesters and who wish to continue in the program(s) they were last registered)
• State-Funded Graduate Assistantship Resignation Form
